چکیده

Introduction: Internal radiation therapy using radioactive Holmium-166 loaded microspheres (HoMS) has shown to be a promising technique for treatment of liver metastases [1]. Unlike Yttrium-90 microspheres, which lack imaging capabilities, HoMS can be detected by Single Photon Emission Computed Tomography (SPECT) using the Holmium-166 photopeak and by MRI using the paramagnetic properties of Holmium. Due to their paramagnetic character, the microspheres induce local field inhomogeneities which cause MR signal dephasing and therefore additional T2 decay [2]. The T2 relaxation time or its inverse, the R2 relaxation rate, can be used to quantify the amount of HoMS [3, 4]. In this paper we report the first results of the detection and quantification of HoMS in patients with liver metastases included in a phase I clinical study [5]. We will demonstrate a) that accurate HoMS concentration maps can be estimated from T2 weighted data, b) that the resulting concentrations maps show good correspondence to activity maps acquired with SPECT and c) that the total amount of HoMS determined using the MR-based concentration maps agrees well with the total amount of HoMS that was delivered.

Multi-echo spin-echo (MESE) signal behavior of paramagnetic Holmium-166 loaded microspheres for radiotherapy: experiment and simulation

Introduction: Internal radiation therapy using Holmium-166 loaded microspheres (HoMS) has shown to be a promising technique for the treatment of liver metastases [1]. Due to their paramagnetic character, HoMS cause additional T2* decay which can be used to quantify the amount of microspheres in the liver using gradient echo sequences [2]. However, for large concentrations of microspheres the si...

Selective depiction of holmium-loaded microspheres (HoMS) using susceptibility gradient mapping (SGM): initial experience in animal models

IntroductionInternal radiation therapy with radioactive holmium-loaded microspheres (HoMS) is a promising treatment option for both unresectable liver metastases and hepatocellular carcinoma. In previous animal studies on internal radiation therapy of hepatic malignancies, MR imaging was shown to be the modality of choice for guiding the administration, planning and follow-up. In particular, T2...

Holmium-166 radioembolization for the treatment of patients with liver metastases: design of the phase I HEPAR trial

BACKGROUND Intra-arterial radioembolization with yttrium-90 microspheres ( 90Y-RE) is an increasingly used therapy for patients with unresectable liver malignancies. Over the last decade, radioactive holmium-166 poly(L-lactic acid) microspheres ( 166Ho-PLLA-MS) have been developed as a possible alternative to 90Y-RE. Next to high-energy beta-radiation, 166Ho also emits gamma-radiation, which al...
